Before filling out this application, Please pay the relevant application fee for your process and be ready with scanned copies following documents.

1. The money should be paid to the account mentioned in the payment letter (See the attachment 1 or attachment 2).

2. Except applicant for a new pharmacy or new wholesale, it is compulsory to mention your file number on your bank deposit slip for our references.

3. Applicant for a new pharmacy or new wholesale should be mentioned the names of the pharmacy and MoH area on your bank deposit slip.

The amount mentioning in the payment letter in Sri Lankan rupees are subject to change monthly due to change of dollar rate. Therefore, you should do both payment and the submission of the online application within the same month.

Documents needed

Make sure you have the scanned copies of the following documents to be uploaded during the application process.

• Scan copy of the application.
• Scan copy of the payment slip.
• Scan Copy of NIC of the responsible pharmacist.
• Scan copy of declaration form of responsible pharmacist.
• Scan copy of declaration form of owner.
• Scan copy of the SLMC certificate.
• Scan copy of the degree certificate/diploma certificate/certificate of efficiency/certificate of proficiency.
• Soft copy of Latest passport size (3.5cm ×4.5cm) colored photographs.
• Scan copy of all supportive documents according to relevant check list that are published at NMRA website.

STEP 2

1. Click the link mentioned at the below to enter the digital application portal.
2. Fill out the application with correct details.
3. Upload the correct scanned documents in the relevant place of the application.
4. Check the spellings and numbers entered are correct and relevant.
5. Ensure to upload soft copy of latest passport size (3.5cm ×4.5cm) colored photographs. (It should be clear and size mentioned above.)
6. Recheck the completeness of the application form before the final submission.
7. If all details are filled and documents are uploaded, submit the application.

At the end of the process, you will receive an email with your submitted information for your reference. This confirms that you have successfully submitted the application form.

Notes: If you fail to complete the application form due to any interruptions or make any errors during the process, you must fill out a new application form.
